St. Croix Hospice provides in-home hospice care through more than 80 branches in 10 Midwestern states, with 24/7 clinical support and specialty care programs.
St. Croix Hospice is a hospice care provider founded in 2008 and led by founder Heath Bartness. The company delivers individualized, in-home hospice care to patients with terminal diagnoses and their families through local teams operating out of more than 80 branches across 10 Midwestern states. Care is provided wherever the patient lives - private homes, nursing homes, and assisted-living facilities - and is available 24/7.
The operational surface is the part that matters here: a distributed clinical workforce moving between patient residences and third-party care facilities, coordinating physician, nursing, and hospice-aide services, medication and equipment coverage, and advance-care planning. As a hospice provider, the organization handles protected health information in a HIPAA-regulated environment, across dozens of branch locations and multiple care settings. Specialty programs - BridgeCare, North Star Dementia, Navigate: Falls Prevention, and Voyage Vigil - extend the care model after admission, while Bereavement Support continues for up to 13 months after a patient's death.
The clinical model is holistic, covering physical, emotional, social, and spiritual needs, with an emphasis on dignity, comfort, and quality of life for patients navigating the end-of-life journey. Local team-based caregiving keeps decision-making close to the patient; any technology supporting that work has to hold up across a 10-state footprint without getting in the way of care that runs around the clock.
